Health coaching and personalized support

Certified health coaches

Comprehensive health coaching and preventive education empower participants to build healthy habits that transform into lasting lifestyle changes. One-on-one health coaching is available by phone at each participant's convenience. Coaching sessions are interactive, personalized and confidential, and can help foster healthy habits around nutrition, physical activity, stress management, sleep hygiene, tobacco cessation and more.

Tel-A-Nurse hotline

Call 877-543-5061 for non-emergency health advice 24/7. Speak with a registered nurse to get guidance on whether your emerging health issue might need the ER, an urgent care facility or a visit with your PCP. There's no copay or cost for using this service.

Neighborly

Neighborly is an online database of up-to-date available social resources, containing resources in every zip code in the United States. Neighborly connects individuals to free and reduced cost programs and services including food, housing, childcare, transportation, utility assistance, education, health care, legal services and financial services. Food is health

Geisinger assists those needing food support with sustainable solutions offered by community partners, such as connection to local food resources, SNAP enrollment assistance, emergency food distributions, and improved food access in rural communities through mobile food pantries to offer a selection of fresh and health food to communities and reach those in need across a broader footprint.

Diabetes and other diet-related illnesses are serious concerns across communities. Geisinger's Fresh Food Farmacy® offers resources for no-cost nutritious foods and health education for members who clinically quality with the goal of better managing their disease condition.

Transportation

Geisinger assists qualifying members in enrolling in transportation benefits. We focus on creating innovative solutions to solve for transportation needs that result in reduced barriers to medical appointments as well as additional locations that address social needs and contribute to overall health.

Housing

The need for housing assistance, or housing-related needs, such as utilities, is often one of the top needs in our member population. Geisinger recognizes the importance of affordable, safe, accessible housing and the impact those resources have on health outcomes as well as quality of life, and provides connections to resources to assist.

Chronic disease prevention and management workshops

Workshops on different health topics including chronic disease management, diabetes prevention and tobacco cessation.

Lifestyle Reboot Program

For people with prediabetes or at risk of developing diabetes. The Lifestyle Reboot Program helps participants:

  • Lose weight and increase physical activity
  • Make healthy choices with the help of lifestyle coaches
  • Reduce the risk or delay the onset of Type 2 diabetes
Live Your Best Life

Managing chronic illnesses is essential for maintaining your health. This program helps you learn how to improve your quality of life while living with a chronic or long-term condition. Family members, spouses, friends or caregivers are welcome to attend with you.

This evidence-based program covers things like:

  • Relaxation techniques

Live Your Best Life with Diabetes

An extension of the "Live Your Best Life" program, focusing on diabetes management and offering more information on:

  • Lifestyle changes to lower glucose and A1c levels
  • Blood sugar monitoring
  • Medication education
  • Skin and foot care
Freedom From Smoking®

A program offered in partnership with the American Lung Association to help tobacco users develop a plan of action that leads to their quit day. The program features:

  • Medication guides
  • Lifestyle changes for easier quitting
  • Stress management and weight gain prevention
  • Long-term solutions to stay smoke-free

A program aimed at increasing activity levels and managing falls. Participants meet weekly to:

LIFE Geisinger

This program supports adults 55 and older, helping them live independently in their home while receiving comprehensive daily living and health services.

GEDWorks™

Geisinger offers financial support if GHP members would like to obtain a GED® in Pennsylvania. The program is offered to Geisinger members at no cost and includes tools needed to pass the GED® test, including free test and study materials and a personal advisor to provide guidance through the program.
